Pear Suite Raises $7.6M in Series A Funding

Pear Suite, a digital health company empowering community health workers (CHWs), today announced that it has raised $7.6 million in Series A funding to advance the development of its AI-powered platform, fuel its provider network growth, and expand its impact through new Medicaid and Medicare health plan contracts. Transforming the role of CHWs in the healthcare system, Pear Suite helps health plans scale culturally responsive, trust-based care interventions that improve population health. The investment round is led by Rock Health Capital and Nexxus Holdings, with support from Mucker Capital, Enable Ventures, The SCAN Foundation, Acumen America, Impact Engine, and the California Health Care Foundation, signaling diverse endorsement and industry confidence.

The funding announcement comes at a time when federal and state policy debates around Medicaid redetermination, work requirements, and preventative care reimbursement are rapidly evolving, carrying the potential to reshape how population health is financed and delivered. CHWs have emerged as a proven, ready workforce by building trust, fostering cultural alignment, addressing health-related social needs, and closing critical care gaps. Health plans increasingly rely on CHWs to improve health outcomes, especially among Medicaid members, older adults, people with disabilities, and other communities that have historically faced systemic challenges in accessing care.

“CHWs utilize their lived experience to provide culturally relevant health education, navigation, and advocacy that addresses the root causes of poor health outcomes,” said Colby Takeda, co-founder and CEO of Pear Suite. “Pear Suite isn’t just a technology company—we’re leveraging our platform to build a national provider network and deliver the contracting, clinical, and claims infrastructure that community-based providers need to thrive. With this financing, we plan to 10x the size of our provider network, continue expanding our reach into the Medicare market, and cement CHWs as a foundational part of the healthcare system.”

Launched in 2021, Pear Suite’s purpose-built platform and wrap-around services help CHWs and other frontline public health workers boost performance and unlock reimbursement. By connecting community-based providers with health plans, Pear Suite advances a skilled and scalable model of care that improves access, strengthens coordination, and supports long-term sustainability.

“The potential impact of scaling community health services is massive,” said Sean Day, Venture Principal at Rock Health Capital. “With a large workforce and reimbursement channels already in place, Pear Suite is the necessary puzzle piece to unlock sustainable funding models for community-based organizations nationally. And the need for Pear Suite’s infrastructure is only becoming more urgent amidst ongoing policy changes. Colby and Nick have done a fantastic job building a flywheel among payers and community health workers that’s creating meaningful impact for patients across the country. We’re thrilled to partner with them.”

Pear Suite began by providing CHW support during the COVID-19 pandemic, helping thousands of families access health and social services in a culturally-sensitive, tech-enabled way. Since then, Pear Suite has achieved extraordinary national growth, supporting more than 300 provider organizations and empowering over 2,500 CHWs, promotores, doulas, and other frontline workers to address critical care gaps.

Pear Suite has also partnered with over 25 health plans to redesign community-based care, align payers and providers, scale value-based models, advance health equity, and build a more resilient healthcare system. The funding will continue to fuel the company’s expansion, transforming health outcomes by uplifting the community workforce.

“Community health workers play a vital role in promoting health and well-being, especially in communities historically underserved by our healthcare system,” says Veenu Aulakh, Director of Acumen America. “We are investing in Pear Suite because their platform gives CHWs the infrastructure to deliver sustainable, culturally rooted care at scale—creating systemic change and a more equitable healthcare system for all.”
