The company intends to use the funds to accelerate the growth of its care model, scale its operations, and extend its reach and impact nationally.
Led by Co-founder and CEO Ahzam Afzal, Puzzle Healthcare provides physiatry-led clinical services and longitudinal care management within skilled nursing facilities to support patients during the transition from hospital discharge through recovery. The company’s service model helps mitigate this risk by embedding care coordination and physiatry services into SNFs, supporting enhanced post-acute outcomes and helping providers avoid reimbursement cuts.
