OffPlan, an Atlanta, GA-based provider of a healthcare platform supporting employers' and employees' access to primary care, has raised $2.5 million in a Seed funding round.
The company plans to use the funds to support its launch and expand into multiple states over the next two years. It is building a new membership-based healthcare model focused on affordability, easier access to doctors, and less reliance on traditional insurance systems.
The company believes that about 80% of healthcare use is routine and predictable care that can be managed more efficiently outside traditional insurance systems. By separating everyday care from major or catastrophic coverage, OffPlan aims to lower costs, improve patient outcomes and reduce administrative work for doctors and employers.

OffPlan focuses on physician-owned practices and offers easier access to care, including same-day or next-day appointments, longer visits, and virtual consultations. It also supports chronic disease management and uses clear, upfront pricing, without the traditional insurance billing and claims process for primary care.
OffPlan was founded to solve what its leadership sees as a core problem in healthcare: using insurance for routine, everyday care. The company combines membership-based primary care, clear pricing for specialty services, and coverage for major health events to make healthcare simpler and more affordable for both employers and employees.
"Healthcare has become increasingly unaffordable because the system treats everyday care like a catastrophic insurance event," said Greg Rable, Co-Founder and CEO of OffPlan. "We built OffPlan to create a more rational structure — one where primary care is accessible, transparent, and centered around the physician-patient relationship rather than claims processing and administrative friction."
"Our goal is not simply to improve the current insurance system," said Omar Fernandez. "It's to build a fundamentally better operating model for everyday healthcare — one that restores physician autonomy, improves patient access, and gives employers a sustainable alternative to annual double-digit cost increases."
