Ilant Health, a New York City-based developer of an AI-supported precision obesity and cardiometabolic care platform, has raised $15 million in a Series A funding round led by Cornucopian Capital.
The round also saw participation from naturalX, Peakbridge, Semcap AI, Evidenced, Operator Partners, Celtic, LifeX, and AlphaLab. The company has raised $22 million in total funding.
The company plans to use the funding to expand its value-based care platform, enhance its data-driven clinical prediction technology, and speed up deployment for large employers and health plans looking to reduce rising pharmacy costs.
Ilant works directly with employers and health plans to provide clinician-led care for obesity and related health conditions. Unlike solutions that focus only on treatment access, cost control, or weight loss alone, Ilant takes a broader approach to improving overall health while delivering measurable clinical and financial results.

Early results show that members lose an average of 15% of their body weight, significantly higher than the 5.8% typically seen in real-world settings. Members also experience improvements in overall health, wellbeing, and key health measures, including an average of two more mentally healthy days each month. At the same time, Ilant helps employers and health plans manage treatment expenses and reduce overall healthcare costs.
The company achieves these results through clinician-led care that is tailored to each member’s individual needs using clinical, behavioral, and personal data. Treatment plans are continuously adjusted based on how each person responds over time.
Its care model includes behavioral therapy, medications such as GLP-1, and non-GLP-1 treatments, and surgical option's when appropriate. The program also addresses nutrition, physical activity and stress management alongside medical care to provide comprehensive support.
Ilant is already partnering with major employers and healthcare organization's to improve patient outcomes while lowering costs. The company is seeing strong demand for its integrated, outcomes focused care model, which goes beyond traditional point solutions by providing more comprehensive and coordinated care.
“Employers and health plans are facing a real dilemma right now — they are faced with either expanding access and watching pharmaceutical costs rise dramatically, or restricting access and risking rising chronic disease and cost,” said Elina Onitskansky, Founder and CEO of Ilant. “That’s not a sustainable model. If we don’t change how care is delivered, we’re just paying more for the same broken system, and we risk a crisis where both chronic disease and costs are spiraling out of control. Ilant’s model focuses on delivering real value with manageable treatment costs and clinically meaningful care that actually bends the cost curve.”

“We believe the next generation of category defining companies will be those that deliver outcomes as a service, not just tools or access,” said Aryeh Ganz, Founder and Managing Partner of Cornucopian Capital. “Ilant is applying that model to one of the most important and costly areas in healthcare. The company understands that the future of obesity and cardiometabolic care will not be defined by access to a single class of drugs, but by the ability to deliver the right care to the right patient at the right time. Their model aligns clinical rigor with economic value in a way that we believe will define this category over the next decade.”
